Manager's Book

Our Affiliate Registrar Susan Cardasis put this book together for our Team Managers.  While the front cover indicates the book is for the Affiliate & Association Registrars, the book was actually developed for them to pass on to the Team Managers in their state.

This book contains the information you need to know as Team Manager when dealing with things like rosters, travel permits, coaching education requirements and credentials. 

Team Rosters

Team Rosters must be processed before any league, non-league, or tournament games are played. 

Your Association's Bylaws, Policies, and Point of Contact list for 2008-2009 Season must be submitted via electronic means to the CAHA Secretary before any teams can be rostered.

Travel Permits - United States Travel 2011-2012

Travel Permits cannot be approved until a Team is legally registered with CAHA and USA Hockey.

Travel Permits must have a USA Sanction Number prior to approval.

Travel Permits must be submitted via email using the form below and should be submitted at least 15 days prior to the travel date.

Word File - To process the word file, you will need to (1) download the file and save it on your system (2) open the file in 'Word' and complete all the information by clicking on the designated fields (3) save the file and (4) send the file as an email attachment to icewitch@comcast.net

PDF File - You can process the PDF file two ways..we recommend option 2:   1 - Open the file, complete the data and click the submit by email button.  Note, some have trouble with this option if they do not choose the correct email option based on their system setup so we recommend using this following option.   2 -  Save the file to your system, complete the information, save it again, and send as an email attachment to icewitch@cocmast.net

DO NOT SEND JPG FILES - THEY CANNOT BE PROCESSED!!

Your submitted travel permit will be signed and returned.  If you do not received an approved travel permit within 5 working days, please follow-up with an email.

Travel Permit - Canada and Other Countries

Out of country travel permits must be processed through your Affiliate Registrar.  Contact the appropriate Registrar for the required documents.
